Replacing a Lost, Stolen or Damaged Driver's Licence

Drivers In Ontario

To replace a driver's licence, visit a Service Ontario Driver & Vehicle Licence Issuing Office. You will be required to provide proof of identification showing your legal name, date of birth and signature, and pay a $10 fee. You will receive a temporary licence to use until your permanent one is mailed to you.  Please refer to the DriveTest website at http://www.drivetest.ca/en/license/HowToApply.aspx for a list of acceptable identity documents.

Effective January 1, 2005, all personal cheques must be certified unless payment is sent by mail.

Drivers Outside Ontario

If you are out of the province but will be returning to Ontario within the next six months, the ministry can mail you a temporary licence, provided that your driver's licence is valid and not cancelled or under suspension. When you return to Ontario, please visit a ServiceOntario Driver & Vehicle Licence Issuing Office to replace your driver's licence. You will be required to provide proof of identification showing your legal name, date of birth and signature, and pay a $10 fee.   Please refer to the DriveTest website at http://www.drivetest.ca/en/license/HowToApply.aspx for a list of acceptable identity documents.

If you will be away longer than six months, the ministry may replace your driver's licence with the current photograph and expiry date, provided it is valid and not cancelled or under suspension.

In order to mail you a temporary licence, we require:

  • a signed letter indicating:
    • your driver's licence number (If you do not know your driver's licence number, we need your full name as it appeared on the licence and your date of birth);
    • the length of time you will be away; and
    • your out-of-province mailing address and telephone number
  • the fee of $10.00 in the form of a cheque or money order made payable to the "Minister of Finance/MTO" and drawn on a Canadian bank account.
  • Note: You may wish to report your stolen or lost driver's licence to the police.

If you wish to use VISA, MasterCard or American Express to pay for your licence replacement, we require a signed and dated letter of authorization that includes the following information:

  • Name of card holder
  • Card type (VISA, MasterCard or American Express)
  • Card number
  • Expiry date (month and year)
  • Reason (i.e. Driver's Licence Replacement)
  • Amount to be charged
