Guide - Notification of Vehicle Brand Form: Individuals or companies who own or lease 9 vehicles or less

  • Individual vehicle owners, individuals leasing vehicles, or companies who own or lease 9 vehicles or less are required to have their vehicles assessed and branded by a licensed Type 6 Motor Vehicle Inspection Station if the individual or company:
    • does not have insurance for the vehicle;
    • has partial insurance for the vehicle, but the insurance policy does not cover the damage; or
    • has insurance for the vehicle, but a claim was not paid within 25 days after the incident.
  • Individuals/companies must notify the Ministry of the vehicle brand by submitting a Notification of Vehicle Brand form indicating the brand of either "Irreparable" or "Salvage" within 26 days of the date of the incident.
  • The vehicle permit must be returned to the Ministry of Transportation when submitting the Notification of Vehicle Brand form.

General Information

  • When completing the Notification of Vehicle Brand form please use black or blue ink that is clearly legible.
  • The form must be completed and signed by the person notifying the Ministry of Transportation of the vehicle brand and the authorized motor vehicle mechanic who inspected the vehicle and is employed by a licensed Type 6 Motor Vehicle Inspection Station1.

1A Type 6 Motor Vehicle Inspection Station (MVIS) is a collision repair facility that is licensed by the Ministry of Transportation and employs registered and certified mechanics who are able to assess the structural integrity of a vehicle.

Completing The Form

Section 1 - Holder of Vehicle Portion of the Permit

  • Indicate your name, surname, address information, driver's licence number/registrant identification number and telephone number.

Section 2 - Registered Plate Owner

(Complete if different from vehicle owner)

  • Indicate your name, surname, address information, driver's licence number/registrant identification number and telephone number.

Section 3 - Vehicle Insurance

  • If you have insurance for your vehicle, complete the insurance company information, registrant identification number, address information, telephone number, fax number, policy number, claim number, and representative.
  • If a claim was not paid to you, indicate "claim has not been paid" in the claim number section.
  • If date of settlement does not apply to you, indicate "Not applicable".
  • Indicate the date of the incident when your vehicle was damaged by collision, impact, fire, flood or theft.
  • Complete all remaining boxes.

Section 4 - Vehicle Information

  • Indicate the 17-digit Vehicle Identification Number (VIN) that is located on the upper left hand corner of your vehicle permit.
    • Note: Ensure that this number matches the one on your vehicle.
  • Indicate the make, model, year, type and colour of your vehicle.
  • Indicate the vehicle permit number that is located on the lower middle section of the vehicle permit.

  • This section must be signed and dated by you the person notifying the Ministry of Transportation of the vehicle brand.
