Guide - Notification of Vehicle Brand Form
Insurance Companies/Self-Insurers
- Insurers as defined in the Compulsory Automobile Insurance Act, are persons/companies who own, lease or hold the plate portion of the vehicle permits for 10 or more vehicles for which the policies of insurance do not cover damage caused by all perils. They are required to determine the vehicle brand and report the brands to the Ministry of Transportation.
- The notification of the vehicle brand is required as follows:
- Insurance companies are required to notify the ministry within six days after the date on which the insurer acquires ownership; or,
- within 14 days after the date on which the insurer pays the settlement, if the insurer does not acquire ownership of the vehicle.
- Self-insurers as described above are required to notify the ministry within six days after the date of the incident.
- Insurers/self-insurers must notify the Ministry by completing the Notification of Vehicle Brand form and indicating the vehicle brand of either "Irreparable" or "Salvage" within 6 days of acquiring ownership
- The vehicle permit must be returned to the Ministry of Transportation when submitting the Notification of Vehicle Brand form.
General Information
- When completing the Notification of Vehicle Brand form please use black or blue ink that is clearly legible.
- The form must be completed and signed by the appraiser and authorized representative of the company.
Completing The Form
Section 1 - Holder of Vehicle Portion of the Permit
- Indicate name, surname, address information, driver's licence number or registrant identification number (if applicable) and telephone number.
Section 2 - Registered Plate Owner
(Complete if different from vehicle owner)
- Indicate your name, surname, address information, driver's licence number or registrant identification number (if applicable) and telephone number.
Section 3 - Vehicle Insurance
- Indicate insurance company information, registrant identification number, telephone number, fax number, address information and policy number, claim number representative and all other sections specified.
- Indicate the date of the incident when the vehicle was damaged by collision, impact, fire, flood or theft, the date of settlement and acquired ownership.
Section 4 - Vehicle Information
- Indicate the 17-digit Vehicle Identification Number (VIN) that is located on the upper left hand corner of your vehicle permit.
- Note: Ensure this number matches the one on the vehicle.
- Indicate the colour, make, model, year and type of the vehicle.
- Indicate the vehicle permit number that is located on the lower middle section of the vehicle permit.
Section 5 - Appraisal/Inspection Contact Person
- This section must be completed by the appraiser or authorized representative who has inspected the vehicle.
Section 6 - Brand Determination
- This section must be completed and signed by the appraiser or authorized representative who has inspected the vehicle.
Section 7 - Declaration
- This section must be signed and dated by person who is notifying the Ministry of Transportation of the vehicle brand.
Submitting the Form
- The Notification of Vehicle Brand form must be submitted to any of the Driver and Vehicle Licence Issuing Offices in Ontario and the vehicle permit must be returned.
