New Vehicle Information Statement (NVIS)

The Ministry of Transportation (MTO) will provide a supply of up to 50 NVIS to companies that manufacture/import trailers. Companies that require more NVIS are responsible for reproducing their own NVIS from a sample provided by the MTO.

To request one sample NVIS, or if up to 50 are required, we require a written request, on company letterhead, that clearly indicates the following information:

  1. Name and address of manufacturer or importer.
  2. Type of vehicles manufactured or imported.
  3. Quantity of blank NVIS required.
  4. National Safety Mark*. A copy of the National Safety Mark, obtained from Transport Canada, must accompany the request. (Please note that the NSM is only issued to Canadian manufacturers.)

* If the vehicles meet the federal safety requirements, Transport Canada may then issue a Ministerial Authorization authorizing the manufacturer to affix an assigned National Safety Mark (NSM) to the vehicles. Each company gets their own unique NSM.

Requests for NVIS forms are processed in the order they are received but in all cases will be filled within 15 working days. If you have an urgent request for a NVIS, please call the Licensing Administration and Support Office at the above number prior to faxing your request.
