Instructions for Completing the Oversize/Overweight Annual Permit Application Form

These instructions will assist you in completing each section of the Oversize/Overweight Permit Application. Permits are normally processed within 15 days of receipt. In order to avoid delays in obtaining a permit, please ensure the permit request is submitted a minimum of 30 days prior to the requested start date of the permit.

For annual permit renewals, sections 1, 2a, 3 and 6 must be completed and a copy of the previous permit must be provided. If there are changes necessary from the previous annual permit, all sections must be completed.

Section 1.  Declaration

  • Applicant must provide name, company position/title and the date of application.
  • Authorized applicant must provide signature, indicating the applicant has read, understood and accepted the conditions of the declaration and has authorization to make such declaration on behalf of the carrier.

Section 2(a).  Applicant Information

  • Registration No.
    • For US or Ontario plated power units, enter applicant's Commercial Vehicle Operator's Registration (CVOR) number.

      NOTE: Carriers operating Ontario or US plated power units without a CVOR must contact:

    • For out-of-province Canadian plated power units, enter applicant's National Safety Code (NSC) number.
       
  • Indicate the total number of identical permits required
  • Check applicable box if permit is required in bilingual (English/French) format.
  • Enter applicant's company name as it appears on the CVOR or NSC, complete head office address and telephone number, company fax number, contact name, e-mail address (if applicable).

Section 2(b).  Agent Information

NOTE: This section is to be completed by the Permit Agency ONLY, if application is made on behalf of the carrier.

Enter agency's company name and complete address and telephone number, fax number, contact name and e-mail address (if applicable).

Section 3.  Purpose of Application

  • Check applicable box to identify if a new permit, renewal, replacement or an amendment is required. If a renewal or an amendment is requested, provide the number and attach a copy of the previous permit.
  • Indicate the requested start date.

Section 4.  Request Authorizing the Movement of

  • Select on page 2, within one category only, the type of load to be moved under an annual permit. Where load specific is not identified on page 2 (categories), provide details on first page in section 4 the load description, overall width, length, height and weight (if weight is required complete section 5a).
    NOTE: maximum annual combination length is 25 metres, width 3.7 metres on single lane highways and 3.85 metres on multi-lane highways and 4.26 metres in height.
  • Some categories require that the applicant indicate the actual width, length and height of the specific equipment to be permitted.
  • Some categories provide the option to select the weight preferred. Select HTA if overall weight is legal, select Standard Annual (see Section 5b for weight condition) for blanket allowable weight or special weights if specific weights are required. (section 5a must be completed for this category).
  • For self-propelled categories, width, length, height and weight must be provided (section 5a must be completed). Provide make, model, serial number and numbers of axles in the spaces indicated.

Section 5a.  Requested "Special Weights" for Selected Weight Category

Where vehicle specific weights are to be listed on the permit, section 5a must be completed in full. This is generally required for specialized equipment such as mobile cranes, but is also preferred by some carriers where blanket weight privileges (Standard Annual Weights - see Section 5b) are not applicable or easy to interpret.

If requesting special weights, applicant must include unit numbers, plate numbers and vehicle identification number (VIN).

Applicants must then complete the Vehicle Configuration Table. Use metric measurements where applicable.

Metric Conversion: Ontario measurement standards are in metric.

  • To convert imperial measurements to metric, convert measurement to inches and multiply by .0254.
    e.g., 9'6" = (9 x 12)" + 6" = 108" + 6" = 114"
           114" x .0254 = 2.89 metres
  • To convert weight from pounds to kilograms, divide pounds by 2.205.
    e.g., 154,000 lbs. ÷ 2.205 = 69,840 kg

Equipment type: List out, on a per-axle basis beginning at the front of the vehicle, the type of equipment being used. (i.e. tractor steer, tractor drive, jeep dolly, float, booster, etc).

Number of Tires/Axles: Indicate the number of tires per axle (range from 2 to 16 tires per axles).

Mfg. Rated Axle Capacity (in kilograms): Indicate the manufacturer's rated capacity for each axle listed. Permit weights will be based on the submitted axle capacities.

Tire Width (in millimetres): Indicate the tire width (in mm) as embossed on the tire sidewall, on each identified axle.

Tire Rating (in kilograms): Indicate the manufacturer's rated capacity of tires on each identified axle. Permit weights will be based on the submitted tire ratings.

Requested Weight Per Axle (in kilograms): Indicate the requested weight required per axle. Authorized weights per axle will be based on all the information provided.

Inter-Axle Spacing (in metres): Indicate the measurement between the centre of the first axle and the centre of the next axle in the vehicle sequence.

Total Weight Requested: Input the total requested gross weight (value must equal addition of all requested axle weights).

Section 5b.  Requested "Standard Annual" for Selected Weight Category

Standard Annual Weights are provided for carrier information. Permitted weights are generally referenced against those permitted under the Highway Traffic Act (HTA), specifically Regulation 413/05: Vehicle Weights & Dimensions. Carriers should familiarize themselves with this regulation in determining their total eligible weight under this section.

Section 6.  Payment and Delivery Options

Check applicable box to indicate method of payment. All personal cheques must be certified unless payment is sent by mail.

When paying by credit card, you must complete the Credit Card Authorization Letter (PDF - 26 KB) and send by fax or mail to the Ministry of Transportation (MTO) Permit Office where you submitted your application.

To protect financial information, please do not send the Credit Card Authorization Letter electronically.

NOTE: MTO permits are only valid on King's highways. The carrier is responsible for acquiring all municipal permits before commencing a move.
