Instructions for Completing the Single Trip Permit Application Form

These instructions will assist you in completing each section of the Oversize/Overweight Permit Application. When applying for a single trip permit, the carrier should apply, at a minimum, 24 hours prior to the proposed move date but MTO suggests applying 2 to 3 days prior to the move to allow sufficient time to process the permit request. Larger loads are subject to additional processing requirements. See the Guide to Oversize/Overweight Vehicles and Loads in Ontario for suggested lead-time in applying for permits.

Section 1.  Declaration

  • Applicant must provide name, company position/title and the date of application.
  • Authorized applicant must provide signature, indicating the applicant has read, understood and accepted the conditions of the declaration and has authorization to make such declaration on behalf of the carrier.

Section 2(a).  Applicant Information

  • Enter the jurisdiction where the power unit is registered
  • Registration No.
    • For US or Ontario plated power units, enter applicant's Commercial Vehicle Operator's Registration (CVOR) number.

      NOTE: Carriers operating Ontario or U.S. plated power units without a CVOR must contact:

    • For out-of-province Canadian plated power units, enter applicant's National Safety Code (NSC) number.
       
  • Indicate the total number of identical permits required
  • Check applicable box if permit is required in bilingual (English/French) format.
  • Enter applicant's company name as it appears on the CVOR or NSC, complete head office address and telephone number, company fax number, contact name, e-mail address (if applicable).

Section 2(b).  Agent Information

NOTE: This section is to be completed by the Permit Agency ONLY, if application is made on behalf of the carrier.

Enter agency's company name and complete address and telephone number, fax number, contact name and e-mail address (if applicable).

Section 3.  Purpose of Application

Check applicable box to identify if a new permit is required or an amendment must be made, include and attach the permit number to be amended.

Section 4.  Proposed Movement Information

  • Enter the START date of the proposed move and the anticipated END date of the move.
  • "Weekend Travel Requested" may only be marked if within annual dimensions
  • Start date must be within 30 days from the application submission date.
  • Enter the ORIGIN of the move, (complete address is required).
  • Enter the DESTINATION of the move, (complete address is required).
  • Enter all Ontario border-crossing locations, if applicable.

Section 5.  Route Details

  • Enter the desired travel route in full, including the jurisdiction name along with all municipal and county roads. Indicate direction of travel along each leg (e.g., left on Durham Road 21, south on Hwy 11, etc.)
  • Enter the range of kilometres to be travelled on the King's highways for the proposed move. (This applies to overweight single trip permits only).

NOTE: MTO permits are only valid on King's highways. The carrier is responsible for acquiring all municipal permits before commencing a move. The indicated travel route will be verified to ensure that all King's highways listed are correct and that construction activities do not impact the travel route including, width, height or weight restrictions.

Section 6.  Travelling on toll highway 407 ETR

If requesting travel on the 407 Electronic Toll Route (ETR), you must indicate by checking the applicable box and provide the Transponder serial number, tractor and trailer licence plate numbers.

Section 7.  Load Description

  • Check the applicable box to identify if the load will be transported, towed or self-propelled and provide detailed description about the load.
  • If a self-propelled vehicle is being used, provide the make, model and serial number.
  • Check applicable box if load is destined for disassembly and/or scrap. If the selection is yes, provide additional move details explaining why it is preferred to move the load intact and disassemble/scrap at a destination.
  • If applicable, enter the overhang measurements from the centre of the rearmost axle.

Using metric measurements only, indicate:

  • Vehicle (tractor) length,
  • Trailer width,
  • Trailer length,
  • Manufacturer's rated capacity of trailer, and
  • Bill of Lading Number, if applicable.
  • Indicate the length, width, height and weight of the load ONLY.
  • Indicate the overall length, width and height of the load and equipment.
  • Check the applicable box to indicate if gross weight is within Highway Traffic Act (HTA) Legal Limits. For information on acceptable limits, please refer to the Guide to Oversize/Overweight Vehicles and Loads in Ontario: HTA Weight & Dimension Limitations.

Vehicle Configuration Table
(heavy bordered area only required if weight exceeds HTA)

Enter Ministry Issued Configuration Number or complete the Vehicle Configuration Table. (To obtain a Ministry Issued Configuration Number, download the request form Request for Vehicle Configuration Evaluation (PDF - 21 KB), and fill out vehicle specific information as illustrated in the accompanying example sheet.)

Metric Conversion: Ontario measurement standards are in metric.

  • To convert imperial measurements to metric, convert measurement to inches and multiply by .0254.
    e.g., 9'6" = (9 x 12)" + 6" = 108" + 6" = 114"
           114" x .0254 = 2.89 metres
  • To convert weight from pounds to kilograms, divide pounds by 2.205.
    e.g., 154,000 lbs. ÷ 2.205 = 69,840 kg

Directions on completing the Vehicle Configuration Table
(Use metric measurements where applicable)

Equipment Type: List out, on a per-axle basis beginning at the front of the vehicle, the type of equipment being used. (i.e. tractor steer, tractor drive, jeep dolly, float, booster, etc).

Number of Tires/Axles: Indicate the number of tires per axle (range from 2 to 16 tires per axles).

Mfg. Rated Axle Capacity (in kilograms): Indicate the manufacturer's rated capacity per axle listed. Permit weights will be based on the submitted axle capacities.

Tire Width (in millimetres): Indicate the tire width (in mm) as embossed on the tire sidewall, on each identified axle.

Tire Rating (in kilograms): Indicate the manufacturer's rated capacity of tires on each identified axle. Permit weights will be based on the submitted tire ratings.

Requested Weight Per Axle (in kilograms): Indicate the requested weight required per axle. Authorized weights per axle will be based on all the information provided.

Inter-Axle Spacing (in metres): Indicate the measurement between the centre of the first axle and the centre of the next axle in the vehicle sequence.

Total Weight Requested: Input the total requested gross weight (value must equal addition of all requested axle weights).

NOTE: Provide a separate diagram for vehicles in excess of 13 axles.

Section 8.  Registered Gross Vehicle Weight

When an overweight permit is required, the Registered Gross Vehicle Weight (RGVW) must be equal to or greater than the weight requested on a permit application, up to the maximum registration permitted in the jurisdiction where the vehicle is registered. An overweight permit request may exceed the maximum permitted in the jurisdiction where the vehicle is plated.

  • Indicate the Registered Gross Vehicle Weight (RGVW) of the Ontario plated vehicle.
  • Out-of-Province Carriers must check the applicable box to indicate if Ontario is registered on the IRP apportioned Cab Card. If Ontario is registered on the Cab Card, indicate the Ontario registered weight. If Ontario is not apportioned, indicate the registered weight (RGVW) in the jurisdiction where the vehicle is plated.

Section 9.  Payment and Delivery Options

Check applicable box to indicate method of payment. All personal cheques must be certified unless payment is sent by mail.

When paying by credit card, you must complete the Credit Card Authorization Letter (PDF - 26 KB) and send by fax or mail to the Ministry of Transportation (MTO) Permit Office where you submitted your application.

To protect financial information, please do not send the Credit Card Authorization Letter electronically.
