Guidelines for Completing the Special Vehicle Configuration Permit Application

  1. Application For:

    • Check appropriate box for permit requirement. Conditions applicable to each permit type are attached. Use separate application forms if applying for more than one permit type.
    • Provide previous permit number if application is for renewal, replacement, or amendment.
    • Fees are $150 for each trailer to be permitted. An additional fee of $50 is applicable to each permit if legally affiliated companies are to be listed on the permit. For the purposes of this permit application, a legal entity is an affiliate of another legal entity if the legal entity has common corporate officer(s), director(s), or individual(s) including partnerships, as recorded on the Commercial Vehicle Operator Registration system.
    • Enter fee and indicate method of payment or prepaid MTO account number. Credit card payments are accepted by either VISA, Mastercard or American Express. All personal cheques must be certified unless payment is sent by mail.
  2. Applicant Information:

    • Enter applicant's Legal Corporate Name.
    • Enter applicant's Commercial Vehicle Operator's registration (CVOR) Number. All applicants and named affiliates must hold a valid CVOR certificate (CVOR application information is available from 416-246-7166 or 1-800-387-7736). All applicants must also have an operating license if they are providing "for hire" service.
    • Enter Company's Head Office Address in full, Telephone Number, Fax Number, Contact Name and E-Mail Address. Original permits will be sent out by normal mail delivery. Applicants may be forwarded a copy of the permit via either fax ($5 charge applies to fax) or e-mail. Check appropriate box if you wish a copy to be provided by fax or e-mail.
    • List name(s) and CVOR number(s) of any legal affiliates as mentioned above.
    • Applicants and named affiliates must have a Carrier Safety Rating of Satisfactory - Unaudited or better.
    • If more room is required to list additional affiliates, check box provided and attach separate affiliate list.
  3. Equipment Identification:

    • List all trailers to be issued permits of the type indicated in Section 1. of the application.
    • VIN (vehicle identification number) must be the industry standard 17 characters in length.
    • The actual date of manufacture must be provided in the format specified.
    • If applying for either the existing 15.5 m or existing 14.65 m Quad permits, applicant must indicate if the Quad axles load equalize.
    • If more room is required to list additional equipment, check box provided and attach separate equipment list.
  4. Declaration:

    • Applicant must sign the declaration of the application. The applicant must have corporate signing authority.
    • Enter position of the applicant.
    • Date of application.

This original, or a true copy of all pages of this permit must be surrendered for inspection upon demand of a police officer or an officer appointed for carrying out the provisions of the Highway Traffic Act. Yes Yes Yes Yes

The semi-trailer and the tractor-trailer combination must conform to Regulation 32/94 except where the specifications set out below differ or where this permit states otherwise. No Yes Yes No
The semi-trailer must be drawn by a 3 axle (tandem drive) tractor. Yes Yes Yes Yes
The semi-trailer must be equipped with four axles: a front single axle and a rear tridem axle. Yes Yes Yes Yes
The inter-axle spacing between the single axle and the tridem axle must be more than 2.5 metres and not more than 3 metres. The minimum distance from single axle to rear tractor tandem must be 6.0 m or more for semi-trailers with a tridem spread of 3.0 m to less than 3.6 m, or 5.5 m for semi-trailers with a tridem spread between 3.6 to 3.7 m. The single axle must be a self-steering axle and capable of turning 20 degrees in either direction. Number of tires required on self-steer axle may be either single or duals. If the single axle is equipped with single tires, thewidth is imprinted on each of them and is at least:
  1. 365 mm when the axle spread of the tridem axle is 3 metres to less than 3.6 metres.
  2. 385 mm when the axle spread of the tridem axle is 3.6 to 3.7 metres.

The semi-trailer must be equipped with a rear impact guard that meets United States Federal Motor Vehicle Safety Standards in place at date of manufacture. No No Yes No
If the single axle of the semi-trailer is liftable, the semi-trailer may be equipped with manual lift axle controls but may not be equipped with automatic lift axle controls that activate other than when reversing. The tractor may not be equipped with nor shall it contain any device, whether remote or manual, that would allow the liftable axle to be controlled by the driver. No Yes No No
The semi-trailer must be equipped with:
  1. a device that accurately displays the total weight of the trailer axles in kilograms; or
  2. a device and a table or chart, from the combined use of which the total weight of the trailer axles in kilograms may be readily and accurately obtained.

  1. It must have been converted by a company authorized under the Motor Vehicle Safety Act (Canada) to apply a national safety mark or by an authorized dealer of such a company.
  2. That person placed a label adjacent to the original Compliance label indicating revised gross vehicle and axle weight ratings.

A semi-trailer with a tridem axle spread of 3.0 m to less than 3.6 m shall have a maximum allowable weight of 32,000 kg. The single axle shall have a maximum allowable weight of 8000 kg and the tridem shall have a maximum allowable weight of 24000 kg. The semi-trailer must have a length with an external measurement between 16.0 and 16.2 metres, excluding any portion of auxiliary equipment or machinery that extends beyond the front of the semi-trailer and that is not designed or used for the transportation of goods. No Yes No No
